Insurance, Referrals, & Co-pays
Insurance
We accept most major insurances and will verify your physical therapy benefits during or shortly after your first visit. Please bring all insurance cards with you to your first visit so that we can make a copy. If you have questions regarding your particular insurance, feel free to give our offices a call or contact us via email below.
Referrals
Some insurance companies, including Medicaid and Medicare, require a referral from your physician within 30 days prior to your first appointment. If you have a referral, whether required or not, please bring it with you or have it faxed to us at:

Co-Pays
If you have a co-pay, it is the portion that is determined by your insurance plan. Our administrative staff will verify your benefits and inform you of your co-pay by your second visit. We collect co-payments on the day that you are seen.
Additional Information
As of January 1, 2022, consumers have new billing protections when getting emergency care, non-emergency care from out-of-network providers at in-network facilities, and air ambulance services from out-of-network providers. Through new rules aimed to protect consumers, excessive out-of-pocket costs are restricted.
